Physiotherapist - Band 6 Community Discharge

Now recruiting on behalf of our Client for a Band 6 Physiotherapist to work within the community discharge service in Belfast. Full time hours.

Job Summary / Main Purpose:

The aim of the Community Discharge Service is to provide a safe, effective, person-centered approach to the transition of medically fit patients from the acute hospital to their own home / or other community setting. The service works passionately towards a ‘Home First’ ethos, and principle that no decisions about long-term care should be made in an acute hospital environment. The Community Discharge Service offers timely therapy follow up of patients upon discharge either at home or in an interim setting; to provide ongoing assessment +/- short term care, supporting the patients’ recovery whilst determining ongoing needs.

Crucial to this post will be collaborative working and onward referral following assessment, with close links to stakeholders including Unscheduled Care, Intermediate care - Reablement/Rehabilitation Services, Community Services, independent/statutory sector and community/voluntary sector.

This post offers a unique opportunity for Physiotherapists to assist in shaping the development of a Discharge to Assess model by using their clinical/assessment skills, knowledge and experience, and by working collaboratively within community transitional services to achieve the best outcomes for our patients on discharge from hospital.

Suitable applicants must meet the Criteria below:

BSc Physiotherapy or equivalent

AND

HCPC Registered Physiotherapist

Must have previous NHS experience in Adult care, have fitness to work (certificate issued through Keenan) and be willing to undergo an Enhanced Access NI through Keenan Healthcare.
